Weight-based epinephrine dosing for emergency indications.
Anaphylaxis: 0.01 mg/kg IM (1:1000), max 0.5 mg. Repeat every 5–15 min. Cardiac arrest: 0.01 mg/kg IV/IO (1:10,000), max 1 mg, every 3–5 min. Croup: racemic epinephrine 0.5 mL in 3 mL NS via nebulizer. Always verify concentration before administration – 1:1000 vs 1:10,000 errors are lethal.
