Stroke thrombolysis dosing – 0.9 mg/kg.
Dose: 0.9 mg/kg, max 90 mg. 10% as IV bolus over 1 minute, 90% infused over 60 minutes. Must be given within 4.5 hours of symptom onset. Exclude hemorrhage with CT first. Monitor BP closely – maintain < 180/105. Check NIHSS before and 24 hours after. No antiplatelets or anticoagulants for 24 hours post-tPA.
